How to Apply
Eligibility Requirements
Don’t hesitate to reach out to our team to discuss any questions or concerns you may have about eligibility or the application process.
The following eligibility requirements must be met in order to apply for the Leading Laboratories designation:
Hospital-based laboratories with current,
valid laboratory accreditation
Organization in good standing
Any organization receiving “Preliminary denial of accreditation” or “Denial of accreditation” decisions is ineligible to apply.
Application Process
The Leading Laboratories designation application is composed of two cycles: Attestation and Application Submission.
1. Attestation
Eligible organizations submit their attestation form, including evidence of current valid laboratory accreditation, from the Leading Laboratories website. The expected review time is two business days.
2. Application Material Access
After being deemed eligible to proceed, the organization receives online access to all Leading Laboratories application materials.
3. Application Submission
The organization has six months upon opening the application to submit evidence of material – component statements, outcomes, indicators, metrics, and evidence for review. A non-refundable appraisal fee of either $1,500 or $2,500 (based on testing volume) is submitted at the time of final evidence submission.
4. Application Review
The Leading Laboratories team reviews all application components, a process that is completed in less than sixty days.
5. Determination
The applicant is notified of a determination. In the event of a denial, the Leading Laboratories team will provide additional details explaining the denial and recommended focus areas for improvement. If the applicant is approved for designation, a designation fee of either $2,500 or $5,000 (based on testing volume) is collected at this time.
